Campos Mental Health Counseling, PLLC
Effective Date: March 3, 2026
Last Updated: March 3, 2026
Campos Mental Health Counseling, PLLC (“we,” “us,” or “our”) is committed to protecting the privacy, confidentiality, and security of visitors to our website located at www.camposmhc.com (the “Site”). We handle personal information with care, respect, and professionalism.
This Privacy Policy explains what information we collect through the Site, how we use it, and the choices you have regarding your information.
This Privacy Policy applies only to information collected through the Site, including any “Contact Us” form or online submission tool. It does not replace our Notice of Privacy Practices (NPP), which is provided to clients during the intake process for counseling services.
1. Important Note About Online Communication
The Site and any contact forms are not intended for emergencies and are not guaranteed to be secure or encrypted communication methods unless explicitly stated otherwise.
Submitting information through the Site, including through embedded or third-party forms (such as Google Forms or similar platforms), does not establish a therapist–client relationship.
If you are experiencing a mental health emergency, please:
– Call 911
– Go to your nearest emergency room
– Contact the Suicide & Crisis Lifeline by dialing or texting 988
2. Information We Collect
We collect information only when you voluntarily provide it or when your browser automatically transmits limited technical data.
A. Information You Provide (Contact Forms or Online Submissions)
When you complete a contact form on our Site — whether hosted directly on our website or through a third-party service such as Google Forms or a similar secure form platform — we may collect:
– Your name

– Age range
– Therapy type requested or areas of concern
– Preferred meeting format (virtual/telehealth or in-person)

We strongly recommend that you do not include sensitive personal health information (such as diagnoses, medications, trauma history, or detailed clinical concerns) in initial contact forms, as standard email and web-based forms may not be fully secure.
B. Automatically Collected Information
When you visit the Site, certain technical information may be collected automatically, including:
– IP address
– Browser type
– Device type
– Pages visited
– Date and time of access
– Referring website
This information is used for website functionality, analytics, spam prevention, and security monitoring.
3. How We Use Your Information
We use the information you provide solely to:
– Respond to your inquiry
– Communicate regarding potential services

– Provide requested information about our practice
– Coordinate next steps related to services
– Maintain website functionality and security
– Prevent spam, abuse, or automated submissions
We do not sell, rent, or trade your personal information to any third party for marketing purposes.
4. No Therapist–Client Relationship
Submitting a contact form, email, voicemail, or online inquiry does not create a therapist–client relationship.
A therapeutic relationship is established only after:
1. Completion of intake paperwork; and
2. A mutual written agreement to begin treatment services.
Until that time, communications through the Site are considered preliminary inquiries.
5. Confidentiality and HIPAA
If you become a client, your Protected Health Information (PHI) will be governed by our Notice of Privacy Practices and handled in accordance with applicable federal and New York State laws, including HIPAA.
Information submitted through the Site before intake is generally not considered Protected Health Information under HIPAA. However, we treat all information shared through the Site with care and professionalism and use reasonable safeguards to protect it.
6. Email and Electronic Communication Risks
Please be aware:
– Standard email is not encrypted by default
– Web forms may transmit data across multiple servers
– Third-party form providers may process data through their own systems
– Electronic communications carry inherent security risks
By contacting us through the Site, you acknowledge and accept these risks.
7. Cookies, Analytics, and Security Tools
The Site may use:
– Basic cookies
– Performance monitoring tools
– Traffic analytics (such as Google Analytics or similar services)
– Spam-prevention and security tools
Cookies are small files stored on your device. You may disable cookies through your browser settings, though some features of the Site may not function properly.
Analytics services collect general usage data to improve functionality and user experience. We do not combine analytics data with clinical records.
8. Third-Party Service Providers
The Site may rely on trusted third-party service providers, including but not limited to:
– Website hosting providers
– Email service providers
– Form processing platforms (including services such as Google Forms)
– Security and anti-spam providers
– Analytics providers
These providers may have access to limited personal information solely as necessary to perform services on our behalf and are expected to maintain reasonable safeguards consistent with their own privacy practices.
We are not responsible for the privacy practices of external websites linked from our Site.
9. Data Security
We implement reasonable administrative, technical, and physical safeguards to protect information submitted through the Site.
However, no method of electronic transmission or storage can be guaranteed to be 100% secure.
10. Data Retention
We retain contact form inquiries only as long as reasonably necessary to:
– Respond to you
– Maintain business records
– Comply with legal, ethical, or licensing obligations
If you do not become a client, your contact information may be deleted within a reasonable period unless legally required to retain it. If you become a client, records will be retained in accordance with New York State law and professional record-keeping requirements.
11. Your Privacy Rights (New York Residents)
If you are a resident of New York State, you may:
– Request correction of inaccurate information
– Request deletion of non-clinical contact information when legally permissible
– Inquire about how your information is stored and used
Professional licensing laws may require retention of certain records once treatment begins.

This Site is not intended for children under the age of 13. We do not knowingly collect personal information from children under 13. If you believe a child has submitted personal information through the Site, please contact us and we will promptly remove it.
13. Do Not Track Signals
The Site may not respond to Do Not Track (DNT) signals due to lack of consistent industry standards.
